Thank you for your generous decision to make a gift of corporate stock to The Church of the Immaculate Conception.

Once you have selected the stock or mutual fund you wish to donate, please complete and return the form below so we may properly acknowledge your gift for tax purposes.

Please provide the transfer form below to your broker for the electronic transfer of stock or mutual funds. We recommend allowing sufficient time for your broker to process the transaction.
